Itching after a hair transplant is a common and normal sensation of irritation in the scalp during the post-operative healing process. This temporary phenomenon affects a significant majority of patients, with studies suggesting its prevalence can be as high as 80-90% in the immediate weeks following the procedure. While rarely a sign of serious complication, the intensity and duration of itching can vary widely, typically beginning a few days after surgery and peaking around the one-week to ten-day mark. It is a natural physiological response as the scalp recovers, involving nerve regeneration, tissue repair, and the shedding of scabs that form over the recipient and donor areas.
The sensation of itching, medically termed pruritus, arises from a complex interplay of factors. As new skin cells regenerate and existing nerve endings are stimulated by the healing process, histamine and other inflammatory mediators are released. The formation and eventual shedding of scabs, which are dried blood and lymphatic fluid, also contribute to the sensation as they lift away from the scalp. The timeline for scabs to fall off, usually between 7-14 days, often correlates with the peak of itching, as the underlying skin is exposed and healing. While most clinics in Turkey advise against direct scratching, they also recommend gentle washing techniques, often with specialized shampoos, to help soften and remove scabs without force. Realistic expectations include some degree of itching, which should generally lessen day by day rather than intensify.
What commonly goes wrong when dealing with itching often stems from a lack of patient education or compliance. Patients, especially those unfamiliar with the typical healing process, might panic or resort to vigorous scratching out of discomfort. Using harsh products, hot water, or not adhering to the post-operative washing instructions provided by the clinic can exacerbate dryness and irritation, intensifying the itch. In some rare cases, persistent or severe itching, especially if accompanied by increased redness, swelling, pus, or fever, could indicate an infection or an allergic reaction to medication, a bandage, or a product used, which requires immediate medical attention.
