At a cellular level, keloid formation represents an uncontrolled proliferation of fibroblasts, the cells responsible for producing collagen, in response to skin trauma. Unlike normal wound healing where collagen synthesis and degradation are balanced, in keloids, this process is dysregulated, leading to a persistent overproduction of thick, disorganised collagen fibres that continue to accumulate long after the initial injury has healed. This excessive fibrous tissue invades the surrounding healthy skin, causing the characteristic raised, often firm and sometimes shiny appearance, which can be accompanied by symptoms like itching, pain, and tenderness.

For a patient considering a hair transplant, understanding keloids is crucial because any surgical incision, including those made during follicular unit extraction (FUE) or follicular unit transplantation (FUT) procedures, carries a risk of keloid formation in susceptible individuals. While hair transplant incisions are typically small, they are still breaches of the skin barrier. The concern is primarily focused on the donor area, where hundreds or thousands of micro-incisions are made in FUE, or a linear incision in FUT. You should always disclose any personal history of keloids, even if they were from minor injuries, or any family history of keloid scarring. Ask the surgeon if they have experience treating patients with keloid-prone skin and what specific precautions they take. Inquire about their protocol for assessing keloid risk, which might include examining existing scars or a test patch in a less visible area, although this is not standard.

Crucially, ask about the type of surgical technique recommended for you and why. For example, if you are at higher risk, a surgeon might advise against FUT and lean towards FUE due to the smaller wound size, or perhaps suggest a limited FUE trial before a full procedure.
