Scalp necrosis is the death of scalp tissue resulting from an insufficient blood supply, most commonly occurring after hair transplant surgery when too many grafts are placed too closely together, overwhelming the skin's natural healing capacity. This critical condition arises when the delicate network of blood vessels nourishing the scalp is compromised, leading to cellular starvation and eventual tissue death. While the human scalp is remarkably resilient, it has a finite capacity to sustain transplanted follicular units, and exceeding this physiological limit can interrupt the vital flow of oxygen and nutrients.

From a patient's perspective, scalp necrosis manifests as areas of black, dark brown, or scabby skin on the treated area, which are often accompanied by pain, swelling, and an unusual odor. Initially, it might present as persistent redness or blistering, progressing rapidly to hard, dead tissue. This dead tissue will eventually slough off, leaving behind an open wound or an indentation. The primary concern for anyone undergoing a hair transplant is the vitality of their scalp and the success of their procedure; necrosis, unfortunately, means that the affected area will not grow hair, and the underlying skin will be scarred, often requiring further medical intervention or corrective procedures.

Other contributing factors can include excessive tumescence fluid (local anesthetic with saline and vasoconstrictors) pressure, prolonged use of epinephrine (a vasoconstrictor) during surgery, pre-existing vascular conditions in the patient (like diabetes or smoking), or even overly tight bandaging post-surgery.

While serious, scalp necrosis is a relatively rare complication in reputable hair transplant practices, with reported incidences ranging from less than 0.1% to 1% in larger studies. The timeline for its appearance is usually within the first few days to a week post-surgery, when the compromised tissue starts to show visible signs of distress. If recognized early, prompt medical intervention might mitigate the severity, but once tissue death has occurred, the damage is irreversible in that specific area. Healing of the necrotic area itself can take many weeks or even months, often leaving behind noticeable scarring that may require subsequent camouflage procedures, further hair transplant sessions into healthy surrounding tissue, or even skin grafting in severe cases.

What commonly goes wrong, particularly in less experienced or high-volume clinics, is the failure to adequately assess the patient’s scalp vascularity and elasticity, or a tendency to prioritize an unrealistic graft count over the scalp’s safety limits. Turkish clinics, being leaders in hair transplant volume, sometimes face criticism regarding high graft counts; however, many top-tier Turkish clinics maintain excellent safety records by carefully adhering to established guidelines. Mismanagement of post-operative care, such as neglecting early signs of inflammation or infection, can also exacerbate conditions that might lead to necrosis.

Patients considering a hair transplant should rigorously research their chosen clinic and surgeon. Crucial questions to ask include: What is the surgeon’s experience with managing complications like necrosis? How does the clinic assess scalp health and vascularity before determining graft density? What is the clinic’s protocol for post-operative monitoring and emergency contact in the days immediately following the procedure? Ensure the clinic is transparent about potential risks and their mitigation strategies. It is also wise to inquire about the specific graft density they plan for your case and why, and to understand their approach to avoiding over-harvesting or over-packing.
