The recommended position usually involves sleeping on your back with your head elevated, often at a 45-degree angle, for the first 5 to 7 nights. Many clinics provide a neck pillow, sometimes referred to as an "airplane pillow," or instruct patients to use several standard pillows to achieve this elevation. The primary goal is to prevent any direct contact or friction between the treated areas – both the recipient site where grafts were implanted and the donor area at the back or sides of the head – and the pillow or bedding. Simultaneously, this elevated posture helps to reduce swelling, particularly around the forehead and eyes, by encouraging fluid drainage away from the face.

Maintaining this elevated back-sleeping position is paramount because the transplanted grafts are extremely delicate and vulnerable during the initial days post-surgery. They are not yet fully anchored into the scalp and can be dislodged with even slight pressure or rubbing. Tossing and turning, sleeping on your side, or sleeping face down could rub grafts against the pillow, leading to their detachment, which is irreversible and directly impacts density and coverage. Furthermore, pressure on the recipient area can impede blood flow, potentially harming the grafts' chances of survival, while pressure on the donor area can cause discomfort or irritation.

Patients are generally advised to strictly follow these sleeping guidelines for at least the first week, sometimes extending to ten days, depending on the clinic's protocol and the individual's healing progress. While some clinics might suggest a return to normal sleeping patterns after 7 days, others may recommend caution for up to two weeks. Common pitfalls include inadvertently rolling onto one's side during sleep, especially if one is naturally a side or stomach sleeper, or the pillow slipping, causing the head to flatten. This can lead to grafts becoming dislodged, an increase in swelling, or in rare cases, a small amount of bleeding from the delicate recipient sites.

To mitigate these risks, clinics often provide specific ergonomic pillows designed for post-surgical comfort and graft protection. Patients should proactively ask their clinic about the exact duration for which these sleeping positions are required and what specific aids, like neck pillows, they will provide or recommend. It's also beneficial to inquire about strategies to prevent unconscious movement during sleep, such as positioning additional pillows around the body to restrict turning.
