Post-op Cysts (Milia)

Post-op cysts, often referred to as milia, are small, temporary white bumps that can appear in the transplanted area after a hair transplant. Patients typically notice these tiny, pearl-like formations as their scalp begins to heal, usually appearing weeks or a few months after the procedure. They often resemble miniature whiteheads or very small pimples, and while they can be a source of anxiety for patients, they are generally harmless and do not indicate a problem with the success of the transplant. These cysts are a common phenomenon and are part of the normal healing process for many individuals who undergo hair restoration surgery.

These small cysts form when newly transplanted hair follicles, or sometimes remnants of skin and oil glands, become trapped just beneath the surface of the skin during the healing process. As the skin closes over the implant sites, these tiny structures can get enclosed, leading to the formation of a keratin-filled sac. The body's natural response is to encapsulate these microscopic foreign bodies, resulting in the visible white bumps. The size of these milia is usually very small, often less than 1-2 millimeters in diameter, and they can be present in varying numbers across the recipient area, sometimes feeling slightly gritty to the touch.

For a patient, the appearance of these cysts can be concerning, as they might fear it impacts the final aesthetic result or signifies infection or graft failure. In most cases, these milia resolve spontaneously within a few weeks to a few months as the skin continues its natural exfoliation process and the transplanted hairs begin to emerge. Some patients may experience them for up to six months. While it's common to see a few dozen or even a hundred of these tiny bumps across the recipient area, a very dense proliferation might be less common. Rarely, if a cyst is persistent or becomes inflamed, it might require a gentle extraction by a medical professional. However, patients are strongly advised against attempting to pick or squeeze these cysts themselves, as this can introduce infection or damage the newly transplanted grafts, potentially jeopardizing the final outcome.

Complications with milia are uncommon but can occur if they become infected or significantly inflamed, which might happen if a patient tries to manipulate them or if post-operative hygiene is neglected. While typically self-resolving, a clinic in Turkey, or elsewhere, should be able to provide clear aftercare instructions that address the potential for these cysts. A reputable clinic will emphasize good hygiene and advise against picking. Patients should always communicate any persistent or concerning skin changes to their clinic, particularly if a cyst becomes painful, red, or swollen, as these could be signs of a secondary infection rather than benign milia.
