A common misconception is that all hair transplant procedures leave a noticeable linear scar; however, this is only true for FUT, while the more contemporary Follicular Unit Excision (FUE) method leaves tiny, scattered dot scars that are far less conspicuous. The linear scar forms when a surgeon carefully excises a strip of skin, typically 1 to 2 cm wide and varying in length, from the back or sides of the scalp, known as the donor area. This strip contains thousands of hair follicles, which are then meticulously dissected into individual follicular units for transplantation. What commonly goes wrong with linear scars often relates to excessive tension during closure, resulting in a stretched or hypertrophic (raised) scar. Sometimes, a "trap door" or "dog ear" deformity can occur at the ends of the incision if the closure is not perfectly flush. Crucially, inquire about the surgeon's experience with trichophytic closure techniques and ask to see before-and-after photos specifically showcasing donor areas and scars. Understand their approach to managing scalp laxity and tension during the procedure. Patients should also confirm the surgeon's strategy for minimizing scar visibility in their particular case, considering their hair characteristics and desired post-op styling. Finally, clarify the post-operative care instructions for the donor area and what to expect regarding the scar's healing timeline and potential appearance, ensuring there's a clear understanding of what a "good" linear scar looks like versus one that might cause concern.
